I had better success with file_regex as:
^([a-zA-Z\/][^:\n]*):([0-9]+):?([0-9]+)?:? (.*)$
This works better with multiline error messages.
better file_regex
^(\S[^:\n]*):(\d+):(\d+): (.+)
