 I am writing to formally make a Subject Access Request, for a digital copy of information held about me, for which I am entitled to under the General Data Protection Regulations (GDPR) 2018.

 You can identify my records by using the following information:
 •    Name: CHANGEME
 •    Address: CHANGEME

 Please supply the data about me, that I am entitled to, under the Data Protection Act 2019, including:
 •    Confirmation that you are processing my personal data
 •    Copy of any and all personal data
 •    The purposes of which you are processing it
 •    The categories of the personal data concerned
 •    The recipients or categories of receipts that you disclose my personal data to
 •    Your retention period for storing my personal data or, where this is not possible, your criteria for determining how long you will store it
 •    Confirmation of the existence of my right to request rectification, erasure or restriction or to object such processing
 •    Confirmation of my right to lodge a complaint with the ICO or another supervisory authority
 •    Information about the source of the data, where it has not been obtained directly by me
 •    The existence of any automated decision making (including profiling)
 •    The safeguards you provide if you transfer my personal data to a third country or international organisation

 As per article 20 of the GDPR, I am exercising my rights to data portability, I am requiring this data to be in an electronic format, that can be machine read, a paper copy will not be acceptable,

 I look forward to receiving your response to this request for data within one calendar month from this date, as per the Data Protection Act 2019 – I expect the data to be made available at the latest, by TODAY PLUS ONE MONTH PLUS ONE DAY.
